Single requestAccess call to avoid duplicate TCC mic dialogs

Each separate AVFoundation call (authorizationStatus, requestAccess)
can trigger its own TCC prompt, causing 5+ identical mic dialogs.
Reduce to exactly one requestAccessForMediaType call which handles
all states internally. Remove the separate status check entirely.
