• Stop the credential refreshers keeping three of the shell scripts' bugs: have codex-refresh take only an answer that holds a string access_token, so a token endpoint answering 200 {} or null is tried again in a minute rather than hit in a tight loop, say once that an invalid_grant means the login must be redone with codex login on the host and look every 5 minutes without spending another exchange until auth.json's refresh token changes, and have claude-refresh count a credentials file unreadable or without an expiresAt after the run as not moved rather than refreshed (b63483c)
