  • Simplified error handling in the OpenAI response stream by returning stream errors before processing response failure events.
  • Formatted response failures inline with the associated message and error code for clearer diagnostics.
  • Removed the standalone helper used to format response failure errors, reducing redundant code.
  • Ensured trailing content is emitted only after the stream is confirmed successful.
