Migrated to the MCP 2.0 (2026-07-28) server SDK (#99). Replaces @modelcontextprotocol/sdk with @modelcontextprotocol/server ^2.0.0 (plus @modelcontextprotocol/core).

ci.yml now pins permissions: contents: read explicitly rather than inheriting the default token scope.

tsc clean; 12/12 integration tests pass against the built dist/. Published and verified against the registry.
