Preserve special characters in attribute values
- Remove trim() to preserve leading whitespace in HTML
- Only escape double quotes in attribute values (HTML5 allows bare <, >, & in quoted attributes)
- Update XML/DOCTYPE regex to handle optional leading whitespace
- Add tests for special characters and empty attributes
