v0.3.9 — Windows Agent startup fix

Fixes Windows Agent startup failing with EACCES when listening on agent.sock (#1).

• Use a deterministic, per-user-directory Windows named pipe for the Agent control channel.
• Keep Unix domain sockets on macOS/Linux; skip socket-file chmod and cleanup for Windows pipes.
• Handle control-channel disconnects during shutdown and wait for connections to close before cleanup.
• Add IPC lifecycle coverage across Windows, macOS and Linux with Node.js 22 and 24.

Upgrade:

If an older Agent is running, stop it before upgrading. Existing user configuration is preserved.

v0.3.8 — 配置热更新与固定 MCP 入口

配置文件保存后自动热更新，无需手动 reload。支持默认配置及 LOCALMCP_CONFIG、原子保存和连续修改；配置无效或新 MCP 启动失败时保留旧配置。热更新保留公网 URL、正在执行的调用和已有持久进程。

外部 MCP 通过固定入口 list_mcp_servers、list_mcp_tools、call_mcp_tool 发现和调用，保留参数校验、图片和结构化结果。新增 MCP 不再改变顶层工具列表。

升级：npm install -g @daodao97/localmcp@0.3.8，然后在本机终端停止并重新启动 LocalMCP。

从旧的平铺 MCP 工具版本升级时，需要刷新一次客户端工具列表并新建会话，同时更新用户目录中引用旧工具名的 Skill。仅修改 SKILL.md 不触发热更新，仍需从外部终端运行 localmcp reload。

验证：类型检查、构建、15 项测试通过；额外验证了 MCP 移除时仍可完成进行中的调用。
