Fixed a timezone mismatch that could cause pending, unclaimed entitlement rows to expire at the wrong time: created_at was left to MySQL's local-timezone default while the stale-entitlement sweep compared it against UTC. On servers not configured for UTC, this could expire rows too early (blocking recipients before their grace window ended) or too late (holding a paid slot longer than intended). created_at is now stamped explicitly in UTC from PHP, consistent with every other timestamp column in the table.
