README cleanup, no functional changes.
- README now focuses on what the plugin does and how to set up a backend (local or OpenRouter) — the discovery/rollup/tracking/memory internals moved to ARCHITECTURE.md for anyone who wants the details
- Plugin description trimmed to drop internal implementation detail
- Fixed a couple of leftover README artifacts (a misplaced screenshot reference, stale placeholder text on screenshots that are already real)
