I don't have access to this Google Form at https://docs.google.com/forms/u/0/?tgif=d. I'd like to move it to a shared location (or create a new form in the DDEV Google Workspace) before pulling it in. |

Even the people who proposed this didn't respond so that's why it died. Not sure it has any future

The Issue
The form looks odd on dark background and there's no link in footer.
How This PR Solves The Issue
Adds white background, removes scroll, adds link in footer.Removes the page.
