gh-discussions: GitHub Discussions as native Tools (search / create / comment, approval-gated)

gh-discussions is a composition plugin that exposes GitHub Discussions as native model Tools: gh_discussion_search, gh_discussion_create, gh_discussion_comment. It's a thin wrapper over gh api graphql (Discussions has no first-class gh subcommand).

I built it after filing #4747, #4796, #4797 and #4799 by hand with gh api graphql all session — wanted the agent itself to be able to search/report/discuss without me shelling out each time.

Design note that might be useful to others building plugins with side effects: gh_discussion_create and gh_discussion_comment publish publicly, so both go through ctx.approval.request() (@deepseek-ai/dsh-user-approval) before executing — a real human confirmation, not a sandbox-mode escalation like kdd-gates uses. Verified fail-closed: run under the headless profile (no answerer configured), gh_discussion_create correctly returned unavailable and published nothing. gh_discussion_search is read-only and skips the gate.
