Docs cleanup, clean package contents, Docker image fix.

Semver rationale: patch — no CLI or API contract changes; fixes a broken Dockerfile and pre-commit hook entry, removes maintainer records from the npm tarball, refreshes docs.

Fixed

• The Dockerfile copied the removed profiles/ directory and omitted document-types/ and personas/, so the image did not build; it now builds and runs.
• .pre-commit-hooks.yaml passed the unknown --gate option to patina-score; it now passes --score-threshold.
• --help lists exit 4 (--verify floor miss / dropped-number guard) alongside the other exit codes.
• docs/API.md renders {@link} symbols as in-page anchors instead of broken links.

Changed

• package.json files excludes docs/operations/**: the tarball no longer ships maintainer launch records (447 → 399 files). Seven maintainer-private records moved out of the public tree.
• Docs refreshed against the shipped 8.1.0 surface: EXIT-CODES, COOKBOOK skip-patterns, INSTALLATION backends, FLAG-PARITY, ARCHITECTURE, release/docker guides, ROADMAP (Polar-only payment, serving engines, phase statuses), a new docs/operations/README.md index; 11 stale docs removed; docs/research/humanization-literature-2026-09.md added.
