Add diataxis-style documentation for the external directive protocol to
cmd/htmlc/README.md, including a how-to guide for adding an external
directive to the build and a full reference section covering discovery
rules, lifecycle, protocol, request/response envelopes, and error handling.
Update the table of contents accordingly.
