Behavioural changes

• Doc::set_block(BlockKind::Heading(_)) over a
  Markdown paragraph of several lines writes one heading with its lines
  joined by spaces (a\nb becomes ## a b), where it made a heading of
  the first line and left the rest a paragraph. A paragraph holding a
  hard break is refused, and the refusal lands in status.
