Fixed

• The uv install path asks the index rather than its own cache. uv keeps an index
  cache with a different lifetime from PyPI's, and the packaging guard reads PyPI's. Measured
  on 0.2.2: the simple index served the file immediately while uv tool install docket-ai==0.2.2 resolved no version of docket-ai==0.2.2 until --refresh was passed.
  The test now passes it. This fix is not provable by mutation here: removing both flags
  leaves the test green, because the cache has since populated and the failure window has
  closed. The evidence is the direct measurement taken in the window.

No change to the shipped code: git diff 0.2.2..0.2.3 -- src/ is empty.
