Recognize ActivityOutcomeRejected diagnostics in history pages, timeline inspection, exports and cold replay while preserving business command positions. Diagnostic summaries use the existing UTF-8 storage bound and tolerate malformed fields without rewriting history payloads.

This is the reader prerequisite for #878 / #901. It does not produce the new event or accept late outcomes. Update and qualify participating applications, Server roles, inspection tools and rollback artifacts, then normal SDK and Waterline readers, before enabling a separate producer. The current portable schema/catalog and closed-run terminal-history decisions remain prerequisites for that producer.
