• Ignore stored Iroh direct addresses during normal reconnects and dial with stable node identity plus relay only.
• Use fresh host.info direct addresses only for controlled relay-to-direct upgrade reconnects.
• Prevent repeated relay upgrade reconnects after a session has already reached a direct path.

• 仅在当前 host.info 返回新 direct 地址时，执行受控的中继到直连升级重连。
• 当当前会话已经到达直连链路后，避免后续中继回落触发重复升级重连。
