Self-Healing Doctor (skillpm doctor)
The doctor command has been revamped from a read-only diagnostic into an idempotent self-healing tool — think go mod tidy for your skill environment. Run it anytime; it detects drift and auto-fixes.
7 Checks (dependency order)

| state | Resets corrupt state.toml to empty |
| installed-dirs | Removes orphan directories and ghost state entries |
| injections | Removes stale injection refs to uninstalled skills |
| adapter-state | Re-syncs adapter's injected.toml with canonical state |
| agent-skills | Restores missing skill files in agent directories |
| lockfile | Removes stale lock entries, backfills missing from state |

Breaking Changes
--enable-detectedflag removed — This behavior is now absorbed into the config check. Just runskillpm doctorwith no flags.
