Fixes

• Claude: "sign in" instead of "no plan rate limits" when there's no sign-in. A machine that isn't signed in to Claude Code (never was, or signed out since) used to get the API key / Bedrock / Vertex message. The helper now only blames a third-party setup when one is actually configured (provider/API-key/gateway env vars, or an env block / apiKeyHelper in settings.json); otherwise it asks you to sign in.
• Gemini is hidden when the account has no Code Assist project, the same as when it isn't set up or has no license.

What's new

• Codex free resets. When Codex grants you rate-limit reset credits, the Codex card shows how many you have and when the first expires, with a Reset now button that redeems one (after an inline confirm) and refreshes the bars.
• Dropdown polish. Clearer row hierarchy (labels in primary ink, larger percentages, a more defined bar track), model-specific pools presented as proper sections, and errors/setup hints shown as compact callouts instead of raw text.
• Gemini is hidden when the account has no license, the same as when it isn't set up on the machine.
• Update notice is now a card with a proper Update/Download button that reads correctly in dark mode.
• On plans with a single usage window (e.g. an Enterprise dollar budget), the menu-bar Style option is named Percentage used rather than "5hr and weekly".
