doc_as_upsert support

Just recently, ElasticSearch added a new feature called doc_as_upsert. If the document does not already exist, it acts as an insert/index, but if it does then it behaves as if you specified the document as upsert. Any chance this could be added as a feature to NEST?

i missed that, that is an awesome feature. support for it should be easy to add. Expect it in the next release :)

Is there a reason ElasticClient.Update isn't asynchronous like Index/Search/Delete?

No other then lack of time and effort.

Historically the idea behind IElasticClient was that each call would have a *Async Raw and RawAsync variant, this proofed to be a bit of a maintenance issue. All the raw calls have since been pushed to IRawElasticClient (with async variants) and I'm now working on stabalising the IElasticClient interface before sitting down and giving each call an async counterpart.

It's high on my on my to do list though.
