• Remove duplicate Instruct and Query prefixes from PREDEFINED_PROMPTS

The instruction_template function already adds "Instruct:" and "Query:"
prefixes automatically, so these should not be duplicated in the
PREDEFINED_PROMPTS dictionary values.
