• Header actions: icon buttons are neutral at rest; accent / danger / success colors only on hover, focus, or active
• Review filter chips: selected state paints immediately (optimistic UI); underline uses ::after (no inset-shadow focus glitch)
• Inline review threads: expanded body/filebar stay clipped inside the card border-radius
• Diff review filter: heavy list rebuild deferred so chip UI stays snappy

Fixes

• Empty conversation composer returns to ghost after blur (preview tab no longer keeps surface open)
• E2E conversation composer Opt-hold + Cmd+Enter and ghost mousedown paths hardened
