Ogma v0.4.0

Fallback model — OGMA_FALLBACK_MODEL + the /fallback command set a model that's used automatically when the primary is unavailable (rate limit/outage); the gateway passes --fallback-model. Hardening: the bot now surfaces the real claude error reason instead of a bare exit code, ignores an invalid hand-edited OGMA_EFFORT at startup instead of failing every message, and bin/setup prints a config summary that flags a missing token / allowed chats / claude CLI.
