Add runtime lookup for bundled prebuilds (index.js) with platform/arch
mapping to prebuild folders. Configure CMake to emit addon.node into a
single NODE_ADDON_OUTPUT_DIR and set target output dirs for all configs.
Include prebuilds in package files, add staging/pack and clean scripts,
and document the release flow in README. Ignore prebuilds and artifacts
in .gitignore.
