Fixed

• Review no longer raises a gating warning when an added entity merely shares a
  name and kind with an entity removed earlier in history and the body differs.
  A bare name match is weak temporal evidence: a common surface name recurs
  across a long history, and the earlier removal may be an unrelated namesake in
  another file. Only an exact behavior-fingerprint restore gates; a
  modified-content match is reported as informational evidence.
