* docs: tighten token permission guidance to least-privilege scopes Correct GitLab guidance from api scope to read_repository + write_repository (the plugin only calls repository tree/blob/commit/ branch endpoints, never issues/MRs/CI). Recommend GitHub fine-grained tokens scoped to Contents: Read and write, Gitea write:repository on 1.19+, and add expiration/rotation guidance for all three providers.
