fix: restore kobject_add for sysfs teardown + speed up unload script

- Restore kobject_add() in module exit with NULL parent: after
  kobject_del(), kobj->parent is NULL but kobject_add falls back to
  the kset parent (module_kset -> /sys/module), correctly re-creating
  the sysfs directory. Without this, mod_sysfs_teardown passes NULL sd
  to kernfs_find_and_get_ns which dereferences it -> fatal NULL oops.
  With valid sd, missing modinfo files/param groups are handled
  gracefully by the removal functions (silent or WARN-only).

- Speed up app unload script: replace per-fd iteration
  (/proc/[0-9]*/fd/[0-9]* = tens of thousands of entries) with
  per-process iteration (/proc/[0-9]* = hundreds), using ls -l | grep
  per process. Also replace setsid (may not exist on Android) with
  subshell backgrounding. Add debug logging to
  /data/local/tmp/ksu_unload.log.
