v0.22.0 — reader gets a table of contents

Release v0.22.0

Polish on the reader release: read:: documents become navigable with an on-page
table of contents and collapsible heading sections, and the FILES sidebar inserts
where your cursor actually is.

• read:: on-page ToC + collapsible heading sections (FLO-815, PR #345)
• FILES sidebar interaction layer: read:: insert, sort, metadata filter (FLO-799 v1.1, PR #339)
• sidebar cmd-click inserts into the focused pane, not the first (FLO-816, PR #345)
• read:: glob path expansion (PR #342)
• FILES extractor cursors independent + inserts auto-execute (PR #341)
• floatty_curl caller -w no longer poisons retries (PR #344)

Frontend + dev-tooling only. 1516 vitest passing.
