Migrate the "CI infrastructure issues" issue templates to use Github forms

We should migrate the existing new issue template for raising an issue with the CI infrastructure to use Github forms. This allows us to add validations for required fields allowing the quality raised issues to be improved.

Discussed with @stuartmorgan and with the triage team internally and decided that there is no need to migrate this issue template as its use is limited to contributors and there isn't really a problem we would solve by doing so.
