Breaking behavior change

Reminders now require explicit acknowledgment. When a time reminder fires, it stays active until you complete it — either via the macOS ✅ notification button or agentrem complete <id>.

Previously, time reminders with max_fires=1 would silently auto-complete after firing once, even if nobody saw them. That's fixed.

What changed

• coreCheck no longer auto-completes reminders when fire_count >= max_fires
• Max-fired reminders stay active but stop re-notifying
• New hasFiresRemaining() filter applied to all 5 trigger types
• 486 tests passing
