Session detail view (new)

Per-session detail surface in both frontends, summarizing session
phase and surfacing modules, threads, and ITrace from one place.
Modules and threads are surfaced from the agent and persisted on the
session node.

.luma directory document

The project format becomes a directory document, replacing the prior
single-file layout. Adopted in both SwiftUI and GTK; malformed
documents are rejected on open.

ITrace

• Streaming and pagination in the GTK UI.
• Waiting state shown for empty traces.
• Delete confirmation; traces deleted by UUID.
• Header buttons aligned to the right in GTK.
• Unified ITrace concept across host and agent, wired through the
  session node.

Module symbols

• Refined module symbol detail view.
• Anchor hints carried through address actions, with smooth module

• GTK: address menu order matches SwiftUI; session detail refreshes on
  phase changes.
• SwiftUI: defers UI state writes to next runloop; uses .primary
  instead of .tint on iOS.
