Updates the bundled Codex app-server to 0.153.4.

  • Includes upstream fixes for Astra model-picker visibility, the bundled default model, and async-question guidance.
  • Protocol schemas are identical to 0.153.3; no generated model changes were needed.

Validation: make lint, make build, 22 focused release/binary tests, and a live app-server initialize + config/read smoke check passed.
