v1.4.0: Replace stale SQL comments instead of adding duplicates

Keeps one SQL comment per name. Edits it in place when only this action's comments follow it or the SQL is unchanged; otherwise posts a new comment at the bottom and deletes the old ones, including comments from earlier versions. Paginates the comment list. (#36)
