Storify Daily Entry (2026-10-16)
The late part of this 24-hour window reads like a handoff story. Agents often completed their own work, yet publication and queue transitions failed downstream. The clearest case is the ESLint factory: two dispatcher runs reached a failed
safe_outputswork-queue control step, three launched miner/refiner/monster runs failed while snapshotting queue state before their agents could start, and a later refiner run completed its agent job but failed 16 Claim adapter preparation jobs plus safe-output processing. These are distinct failure boundaries, not proof of one root cause.A second pattern crossed unrelated workflows: log redaction failed in Smoke Crush, Matt Pocock Skills Reviewer, and Daily AgentRx Trace Optimizer. Triage had a different boundary: Auto-Triage Issues and PR Triage Agent stopped at the daily token guardrail with their agents skipped, then each had a later successful scheduled run. Coverage is partial: the cached index contains 263 runs from 2026-10-15 23:35:43 to 2026-10-16 07:56:29 UTC. Collection stopped at a count limit; the requested interval from 2026-10-15 07:57:52 to 23:35:43 UTC was not available, and no additional agentic-workflows query tool was exposed here.
Episode Highlights
ESLint queue handoffs stalled. The dispatcher agent and detection jobs succeeded before
Process trusted work queue controlsfailed in §38845385623; an earlier dispatcher run showed the same boundary. Miner, refiner, and monster launch runs then failed at activationSnapshot work queue state, with agent jobs skipped, including §38830731349. A separate refiner execution, §38845844758, got through the agent and detection stages, then failed 16 isolated Claim adapter preparations andProcess Safe Outputs. Outcome: stalled at multiple handoffs. Confidence: high in job and step attribution, medium in any shared explanation.Redaction became a cross-workflow boundary. §38826247553, §38840691314, and §38856386715 each report a failed
Redact secrets in logsstep in the agent job. The first two also had CLI execution failures; the third did not. Outcome: recurring failure, with no common cause established. Confidence: high for the repeated step, low for causality.Triage recovered after early guardrail stops. §38831804314 and §38831888632 failed in activation at
Check daily workflow token guardrail; both agent jobs were skipped. Later §38862104606 and §38862218523 succeeded through agent and safe-output jobs. Outcome: later recovery observed; its cause is unknown. Confidence: high in sequence.Feedback Loops Across Workflows
The Publication-boundary repair loop remains active: queue and safe-output failures produced human repairs, including merged PR #69230 for accepted launch verification and PR #69277 for Claim adapter output loading. Direction: improving prospectively, because the inspected cache has no confirming ESLint run after those merges. A related safe-output failure also appeared in the container image scan §38856398740, but its common cause is unproven.
The Failure-to-investigation closure loop is mixed. The shared redaction step remains a recurring unresolved signal across three engines, while the two triage guardrail stops were followed by successful runs. Do not count skipped agents as agent failures.
Human Interventions That Mattered
Peli de Halleux merged PR #69230 at 05:22 UTC and PR #69277 at 06:41 UTC, after the observed queue failures. Their titles identify targeted changes to launch verification and Claim adapter preparation. The available run sample does not yet establish their effect in production.
Signals to Watch Next
Look for a post-merge ESLint dispatcher-to-worker run with successful queue snapshot, Claim preparation, and safe-output jobs; check whether redaction succeeds across all three affected workflows; and recover the missing early-window run range before making any full-day rate claim.
