• env() sees variables the real process environment holds. The helper read $_ENV alone,
  which PHP fills only when variables_order includes "E" (the php.ini defaults do not) — and
  Dotenv's immutable loader skips any key the real environment already holds. Under a CI job
  or a container that exports DB_*/APP_*, every env('DB_DRIVER', 'sqlite') therefore
  returned the default until the installer published its credentials into $_ENV: a Postgres
  install's first connections (the shared repository connection created at boot, the RBAC
  provider) silently went to sqlite, and a fresh install's role grants failed with "no such
  table: permissions". env() now reads $_ENV, then $_SERVER, then getenv(), with the
  same boolean/null casting for all three. A test that wants a key absent must clear it from
  the real environment too (putenv('KEY')), as LoggingConfigProfileTest now does.
