v0.84.3: the editor lifecycle rounded out

Four editor fixes from live production use, all drop-in (no Consumers must: steps).

Changed

• The editor's Publish button is now always visible in the desk band beside Save, matching the
  convention across comparable CMS editors (WordPress, Ghost, Sanity, Contentful). When an entry
  has nothing new to take live it stays present but guarded (aria-disabled, dimmed, "Nothing
  new to publish"), and it wakes on a typed edit, a pending draft, or a new entry. Previously it
  rendered only after a save had cut the draft branch, which hid the publish step from a first
  draft and forced a save-then-publish two-step the publish action never required.

Fixed

• A brand-new entry now opens with the title typed in the create dialog. The create step
  collected the title but dropped it, so the editor's title field opened blank and the top bar
  showed the address instead.
• A brand-new, never-saved entry's status badge read Published. It now reads New until the entry
  first lands on the live site, matching the list's vocabulary.
• Common English contractions (you've, they're, doesn't, and the rest of the standard set) no
  longer show as spelling errors, and words written with a curly apostrophe (as rich-text paste
  produces) are normalized to their straight-quote dictionary form at lookup.
