State file migration to .prp-output/state/

All state and lock files moved from .claude/ to .prp-output/state/ for tool-agnostic compatibility.

Changed

  • State file location: .claude/prp-run-all.state.md → .prp-output/state/run-all.state.md
  • Lock file location: .claude/prp-run-all.lock → .prp-output/state/run-all.lock
  • Ralph state file location: .claude/prp-ralph.state.md → .prp-output/state/ralph.state.md
  • State file I/O: prompts now instruct Bash heredoc for state writes (avoids permission prompts in Claude Code)
  • Permission config simplified: removed "Bash(rm -f .claude/prp-*)" — .prp-output/* rule covers state files

Why

  • .claude/ is Claude Code-specific — Codex, OpenCode, Gemini, Antigravity adapters were writing to a path that doesn't belong to them
  • Write/Edit tool calls to .claude/ trigger permission prompts, breaking autonomous run-all execution
  • .prp-output/ is already the shared artifact directory across all tools

Migration

  • In-flight --resume from v2.5.x will report "No saved state found" — safe, just re-run without --resume
  • No data loss — state files are transient checkpoint data
  • Permission configs: replace "Bash(rm -f .claude/prp-*)" with "Bash(rm -rf .prp-output/*)" (or remove if already present)
  • Run prp-install-all to update all repos

Stats

  • 51 files changed, 51 bats tests pass (unit + e2e + ralph)
  • All 5 adapters updated (claude-code, codex, opencode, gemini, antigravity)
