govctl v0.20.1

0.20.1 stops agents from fabricating ADR alternatives. The ADR-0042 gate
thresholds required a rejected alternative even for genuinely uncontested
decisions, which produced bloated ADRs full of straw options. ADR-0063
supersedes ADR-0042 and relaxes both gates.

• Writing an ADR decision is blocked only when no alternatives exist, not
  when fewer than two exist or none is rejected. (WI-2026-09-16-001)
• govctl adr accept requires at least one accepted alternative and requires
  a rejected alternative only when two or more alternatives exist.

• The adr-writer skill no longer demands at least one rejected option, treats
  bloat as a defect, and the adr-reviewer flags fabricated or quota-filling
  alternatives instead of requiring them.
• The wi-writer skill bans forward references to symbols that do not yet
  exist, and the wi-reviewer flags them, so work items describe planned
  concepts by role rather than by planned identifier.
