Fixed

• Two references added at about the same time (or added right after Obsidian starts, before it has read every note) could get the same citekey. New notes are now created one at a time, and notes Obsidian has not parsed yet are read directly when a key is chosen.
• MCP add_reference now tags a new note with its PubMed MeSH headings and author keywords (as the PubMed search window does), and keeps the PMC id. Bulk imports through Claude Code / Codex were landing untagged.

Changed

• The key setting is now labelled API key (OpenRouter or OpenAI) (was "OpenAI API key"), so it is clear the default base URL expects an OpenRouter key; searching settings for "OpenAI API key" still finds it.

• User guide: the setup step names OpenRouter and links to its key page, and says an OpenAI key needs the base URL changed first; install instructions search for "Refwright"; every screenshot re-shot with the Refwright name.
