Session termination now removes activity records and pending cleanup timers, fixing stale state accumulation during login rotation, logout, and cache invalidation.

• Clear stale activity after a session process crashes.
• Cancel timers during both scheduled and periodic TTL cleanup without synchronous calls from Cleanup to itself.
• Preserve active sessions and remove stale activity even when no timer remains.
• Update active dependencies within existing constraints, including Plug 1.20.3, and remove unused lock entries.

Includes #59; fixes #58. Validated with 184 tests, strict compilation, formatting, Credo, Dialyzer, documentation generation, and Hex package build. GitHub tests and benchmarks passed before merge.
