fix: use /usr/bin/which to discover heroku path at runtime

Static path checks cover standard installs, but fall through to running
/usr/bin/which (absolute path, always present on macOS) with an augmented
PATH. This handles any non-standard heroku install location without relying
on the GUI app's sparse inherited PATH.
