v0.11.0 — inline right-hand report pane

Clicking a report card opens the report in the same layout plane: the conversation is pushed left and the panel sits inline on the right (Codex-style split, no overlay).
The panel has a Refresh action (re-mounts the report).
When the model produces a newer report in the conversation (e.g. after you send an edit prompt), the open panel auto-replaces its content.
