E8 · ⌘K coverage + agent command

Goal

Close the ⌘K gaps: add the missing commands (open agent — none today; open Connections; open a transaction) and enforce that every v4 verb is one command id that all surfaces dispatch.

Depends on / blocks

• Depends on: E2, E3, E4, E5, E6, E7 (wires ⌘K over the finished destinations/actions).
• Blocks: nothing (last).

Do not touch

• Existing command ids / shortcuts (add, don't rename).

Acceptance criteria

• Every v4 destination/action is ⌘K-reachable (open agent, open Connections, open a transaction, plus all v4 verbs).
• A reflective test asserts registry ↔ handler coverage.
• Palette screenshots attached.
