- Fixed rate limit detection to trigger immediately on first 'out of extra usage' message (was 3+)
- GLM and other API providers now switch to fallback models instantly when hitting limits

- Added current model display on task cards (shows active model like 'glm-4.7', 'claude-sonnet-4')
- Cards update in real-time when switching between models
