🛠️ Improvements

• Automatic fallback model switching when rate limits are detected
• Model badge now shows currently active model (reflects fallback switches)
• Phase-specific fallback chains (uses correct fallback list for planning/coding/qa phases)
• Rate limit detection during execution (not just on process exit)
• Added GLM model support to model labels and colors

🐛 Bug Fixes

• Fixed model badge showing configured model instead of actually running model after fallback switch
• Fixed fallback chain using wrong phase configuration when restarting after rate limit
