othermodel: 修复旧历史无 reasoning_content 时的 400 自愈边界 bug

sanitize_model_turn_segment 在处理缺 reasoning_content 的旧 tool turn 时：
- 若 trailing_assistant_messages 为空，之前返回完整 segment（含无 reasoning 的
  tool calls），导致 DeepSeek 400: "reasoning_content must be passed back"
- 现改为过滤掉所有 tool call/output 项，保留 assistant message 等无问题项
- 同时加入 CustomToolCall 检测，此前遗漏可能导致自愈逻辑不生效
