The problem
Edit because I see there have been some other tickets around this saying Nest doesn't expose the state: The same thermostat is also added to Apple Home over Matter, and it does reflect modes (off/heat) vs state (idle/heating) in the Apple Home app, so Nest must be exposing that information somewhere.
I switched from the Nest integration with an older Gen 2 thermostat to a Gen 4 thermostat over Matter, and on a dashboard I have a graph showing the entity's status (current/target temperatures and heating status).
The old entity's state only showed heating when the thermostat was actually calling for heat, it didn't reflect the mode of the thermostat (off/heat). However the new entity's state is just "heat" all the time, it doesn't reflect heating/idle. The dashboard widget also never gets the background "glow" when the thermostat is actually heating.

I can provide any other logs/matter dumps if needed, I'm just not sure what would be helpful.
