Report the resume delivery path and runtime capabilities

v0.2.12 could still fail silently: when a TUI exposed no session prompt API in the expected shape, the resume returned without a trace, and multiple plugin processes appending one bounded diagnostics file clobbered each other's records.

Fix

  • The resume now probes and supports both client shapes — the current flat session.prompt and the older v2.session.prompt — so it works across OpenCode generations.
  • Every branch records an outcome, including the previously-silent "session prompt API unavailable" case.
  • A plugin_environment diagnostic is written at load with the plugin version and the client capabilities each process can see, making runtime mismatches directly observable.
  • Diagnostic appends are now atomic; the 100-record bound is applied with a soft-cap rewrite, so concurrent plugin instances never lose records.
