feat: improve modifier/layer/combo UI and fix layer id/index bug

- ModifiersTab: remove one-shot mode, add explicit Apply button for all modes
- LayersTab: fix layer param1 to use index instead of id, fix name fallback
- ComboSettings: show key labels instead of position numbers, fix state mutation
- Add modifier keys (Ctrl/Shift/Alt/Cmd) to tap key selection
