Added

• Incremental indexing — qnode index now skips files whose mtime and collection assignment are unchanged since the last run, and forces a full reparse when a collection's field→category config changes. Pass --force to bypass and fully reparse. Files removed from disk now have their node and edges cleaned up automatically, both in-collection and external (edges pointing at a deleted node are unresolved rather than left dangling).
