Fixes Markdown files that open with raw HTML showing up as plain text in the preview.
- Find raw HTML blocks the way GitHub does, so a README that opens with an HTML header, such as ghostty's, renders in full with its headings, links, and tables.
- Stop wrapping a
<details>block around fenced code, an indented<p align="center">, or a bare<br>line in extra paragraphs and line breaks. - Add regression coverage for HTML block boundaries.
Verified with all 48 automated tests and ghostty's README in Sublime Text. Fixes #9.
